Ascension is committed to providing a safe environment for associates, patients and visitors.

In alignment with this commitment, Ascension is requiring all associates, as well as individuals performing work or providing services in our facilities, to receive the COVID-19 and influenza vaccines.The COVID-19 vaccination campaign will run at the same time as the annual influenza vaccination campaign. Both will require associates to obtain vaccines or submit proofs of vaccinations for COVID-19 and flu on or before November 12, 2021. Ascension Associate and Occupational Health (AOH) will offer free vaccine clinics for associates, as well as contingent workers, contractors or other non-employed healthcare providers. COVID-19 Vaccine
    Ascension is committed to providing a safe environment for associates, patients, visitors and the community. In alignment with this commitment, Ascension is requiring all associates to receive the COVID-19 vaccine. This includes employed associates, as well as those employed by subsidiaries and partners; physicians and advanced practice providers, whether employed or independent; students and medical residents; and volunteers and vendors entering Ascension facilities. Proof of vaccination must be submitted by November 12, 2021.

  Requirement information
 

Associates will be required to submit proof of COVID-19 vaccination (see Providing proof of vaccination, below). Proof of vaccination must be submitted by November 12, 2021. Exemption requests must be submitted for review no later than October 1, 2021.

     
    For those with direct reports
    Fulfillment reports will be available soon. Once these reports are available, associates can check with their managers regarding receipt of their vaccine documentation.
     
    For contingent workers or contractors
    Contingent workers, contractors or other non-employed healthcare providers, including independent providers, students and volunteers, are required to meet the same COVID-19 vaccine requirement as Ascension associates. If your employment falls into this category, you should provide your COVID-19 vaccination documentation to your company or school (unless you have been advised by your company or school to submit your documentation to Ascension AOH). Any contingent worker, contractor or other non-employed healthcare provider may receive a vaccine from an Ascension and Occupational Health (AOH) vaccine clinic, where a receipt will be provided to you.

You can receive your COVID-19 vaccine at an Ascension AOH site or an AOH-sponsored event, where your receipt is automatically documented. See the ministry market links at the bottom of this page for Ascension AOH vaccine clinic hours and locations.

You can also receive your COVID-19 vaccine through an Ascension Medical Group physician office, Ascension urgent care location, Ascension Rx pharmacy or a non-Ascension provider or pharmacy and submit documentation of your vaccine by clicking on the "Submit COVID-19 documentation" button below.

     
    Providing proof of your COVID-19 vaccination
 

Associates and others who are vaccinated at an Ascension AOH site or an AOH-sponsored event will already have their vaccination status on record with AOH.

Associates who have been vaccinated through other providers, such as an Ascension Medical Group practice, Ascension Urgent Care, Ascension Rx pharmacy, or a non-Ascension provider or pharmacy, should provide proof of vaccination by clicking on the button below to submit their documentation.

If you have previously submitted proof of vaccination from an external location, you DO NOT need to resubmit.

    Requesting an exemption from the COVID-19 vaccine
 

Even if you were approved for medical or religious exemptions in the past for other vaccines, you will need to resubmit a medical or religious exemption form specific to COVID-19.

Whether you are requesting an exemption for medical reasons, or requesting an exemption due to your sincerely held religious beliefs, you will need to submit your request electronically. Your medical exemption request requires that you provide documentation from a licensed healthcare provider who is providing ongoing medical care to you.     • Requests for exemption are individually reviewed by the ERC. The exemption determination will be communicated to the associate and direct supervisor via email. In keeping with its mission and values, Ascension will not deny or terminate employment based on religion, disability or the request for an exemption.

Influenza Vaccine
    Ascension is committed to providing a safe environment for associates, patients and visitors. In alignment with this commitment, all healthcare personnel (HCP) providing services are required to receive an annual influenza vaccination. Proof of influenza vaccination must be submitted by November 12, 2021.
     
  Requirement information
 

Associates will be required to submit proof of influenza vaccination (see Providing proof of vaccination, below). Proof of vaccination must be submitted by November 12, 2021. Exemption requests must be submitted for review no later than October 1, 2021. The Centers for Disease Control and Prevention (CDC) recommends that people get a flu vaccine by the end of October. However, getting vaccinated early (for example, in July or August) is likely to be associated with reduced protection against flu infection later in the flu season, particularly among older adults.

Associates identified as fully remote, or who work exclusively in non-clinical office locations, are exempt from receiving the flu vaccine as this is our annual process, and influenza is not a pandemic. Associates with this exemption will receive a notification email; there is no action required. For the health and safety of Ascension's workforce, Ascension strongly recommends that all associates receive the flu vaccine regardless of position status.

Should an associate’s remote work status change, associates should contact their local AOH department to make necessary arrangements to receive the flu vaccine.

    The flu vaccine requirement applies to all HCPs providing services at our Health Care Ministries, including independent providers and contingent workers such as contractors, students and volunteers. Any contingent worker, contractor or non-employed healthcare provider who is subject to the influenza vaccine requirement may receive a vaccine from an Ascension Associate and Occupational Health (AOH) vaccine clinic, where a receipt will be provided to you. If your employment falls into this category, you should provide your flu documentation to your company or school (unless your company or school advises you to submit your flu documentation to Ascension AOH).

If you were approved for a permanent medical exemption for the flu vaccine in the past, you do NOT need to submit a new request in 2021-2022.

Associates who were granted permanent religious accommodations in past years must submit a new request to be granted an exemption for 2021-2022.

Whether you are requesting an exemption for medical contradictions, or requesting an exemption due to your sincerely held religious beliefs, you will need to submit your request electronically.
